I am over 45 and suddenly need reading glasses: what are my options?

It tends to happen without much warning. One month the menu is fine. A few months later you are holding it at arm’s length, adjusting the angle, hoping the light is just bad.

Then you are in a pharmacy, trying on reading glasses off a rack, telling yourself it is a temporary measure.

For most people it is not temporary. It is presbyopia, the gradual stiffening of the natural lens inside the eye that makes it harder to focus on close objects as you get older. It affects almost everyone at some point, usually between the ages of 40 and 55, and it is one of the most common reasons people in this age group begin asking questions about vision correction.

Reading glasses are a reasonable response to presbyopia. For many people they are the right choice. But they are not the only option worth understanding, and the alternatives have become more accessible over the past decade. This article explains what presbyopia is, what options exist, and how to think about the decision.

Why this is different from short-sightedness

If you have spent your life wearing glasses or contact lenses for short-sightedness, you may assume that laser eye surgery is the obvious next step. The picture is more complicated after 45.

Short-sightedness is a refractive error caused by the shape of the eye. Laser eye surgery corrects it by reshaping the cornea. Presbyopia is caused by the lens inside the eye losing flexibility, and reshaping the cornea does not address that directly.

This does not mean laser eye surgery is irrelevant after 45. It means the conversation changes, and the options available to you depend on your specific prescription, your corneal profile and your overall eye health.

Presbyond: a laser approach adapted for near vision

Presbyond is a laser procedure designed for patients with presbyopia. It works by creating what is called blended vision: one eye is corrected slightly more for distance, the other slightly more for near. The brain adapts to blend the two images, giving a functional range of vision across different distances.

It is not the same as traditional monovision, which creates a more pronounced difference between the two eyes and which some patients find difficult to adapt to. Presbyond uses a gentler gradient. Some patients adapt well to this approach, while others may find the difference between the eyes harder to tolerate.

Presbyond is not suitable for everyone. Candidacy depends on corneal thickness, prescription, age and other factors that a thorough screening will assess. Outcomes vary between patients, and some patients may still require reading glasses for certain tasks after the procedure. A practitioner will be able to give you a realistic picture of what is achievable for your specific situation.

Lens replacement surgery: a different approach

For patients whose natural lens has already begun to change, or for those whose prescription or corneal profile makes laser surgery less appropriate, lens replacement surgery is another option worth understanding.

The procedure involves removing the eye’s natural lens and replacing it with an artificial intraocular lens. Depending on the type of lens chosen, it can address distance vision, near vision or a range of distances. Multifocal or extended-depth-of-focus intraocular lenses can reduce dependence on glasses at multiple distances, though they are not suitable for all patients and carry their own risk profile.

Lens replacement surgery is a more involved procedure than laser eye surgery. Recovery takes longer, and the decision requires a detailed discussion with your surgeon about your expectations, your lifestyle and what the realistic outcomes are for your individual circumstances.

It is also worth knowing that once the natural lens is replaced, the eye will not develop a cataract in that removed lens later in life, though other age-related eye conditions can still occur. For some patients over 45, this is a relevant consideration when weighing up the options.

What about continuing with reading glasses?

Reading glasses work. There is no clinical reason to pursue a procedure if reading glasses meet your needs adequately. The question is whether they fit your life as it actually is.

If you use them occasionally, for a restaurant menu or a book before bed, they may be entirely adequate. If you spend your working day moving between a screen, a phone and printed documents, the constant reaching and replacing becomes its own kind of friction. If you are active or in situations where glasses are inconvenient, the calculation shifts.

The point is not that reading glasses are a poor choice. The point is that they are not the only option, and the alternatives are worth understanding before you decide.

How to approach the decision

The most useful first step is a proper clinical assessment, not a self-diagnosis based on which reading glasses feel right off a pharmacy rack. A thorough evaluation will look at your corneas, your natural lens, your prescription and your lifestyle, and will give you an honest picture of which options, if any, may be appropriate for you.

It is also worth approaching the decision without urgency. Presbyopia progresses gradually, and there is generally no clinical reason to act quickly. Taking time to understand your options, speak to a practitioner and consider what matters most to you is a reasonable approach.
